Stratified Self-Managed Follow Up Framework

NCCP Cancer Survivorship Stratified Self-Managed Follow Up Framework aims to adopt a standardised approach nationally to stratified follow up care after cancer treatment with a more personalised approach for cancer patients.

Patients needs are assessed holistically, taking into account physical, psychological and social factors, rather than just the symptoms of a disease.

This Framework has been developed to support services in the design, development and implementation of these new follow up pathways.
